Register with us as a New Patient

Accountable/Named GP for All Patients

All patients registered at Sydenham Green Group Practice have a named doctor who has overall responsibility for your care and support. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

How to register 

Register Online

The quickest way to register is by completing the online form below:

You can register at the practice using the forms below. Please do not come into the practice unless you are unable to register online.

You will need.

  • Your name, date of birth and NHS number (if you know it).
  • Your town and country of birth.
  • Your previous address.
  • Your previous doctors name and address
  • If you are already registered with a GP your NHS medical card.
    If you haven’t got your card your records can be traced and we will ask for:
  • Proof of address showing that you live in the practice area.
    This may include: Utility bill, Phone bill (NOT Mobile), Bank statement, Driving licence, Rent agreement.
  • Complete a brief questionnaire for each member of the family.
    It is very important that you complete this as it may take some months for your medical records to reach us.
  • Complete details of your and your family history medical history as well as blood pressure, height and weight on our Surgery POD within the waiting room.

Patients are accepted purely on eligibility and address and not on their health, age, gender, religion or other criteria. If your application is accepted you may be asked to make an appointment with a nurse for a check-up. This allows us to check a few things like your blood pressure and to make sure we have a picture of your previous medical history and also just to say hello! Please bring a urine sample with you.

PLEASE NOTE:

  • Visitors to the area and eligible for NHS care may be accepted as ‘temporary residents’ for a limited period.
  • Overseas visitors are treated on an NHS basis for ‘immediately necessary’ medical emergencies only.
  • We do not see private patients

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

Step 1: Download and complete a registration form

Please complete the General Medical Services (GMS1) Form and Adult/Child Registration Form for each new patient.

Step 2: Upload completed registration form and evidence of identity & address

Please deliver to the practice completed registration form and copies of two forms of your identity.

Doctor Preference

When registering with the practice you have the rights to express a preference for the doctor you would like to see and this information will be kept in your records. We are however unable to guarantee that a particular doctor will be available when required and if not we will endeavour to provide a suitable appointment with another practitioner.

The National Care Record Service (NCRS)

Important information regarding your NHS medical records – Summary Care Record

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
