What is the Patient Forum?
It is a group of volunteer patients who are involved in discussing health issues and helping shape the services to patients.
Will the questions you ask me be medical or personal?
They will be general questions about the practice, how we are providing services and what we can do to improve them. Questions about your personal care or complaints will not be dealt with through the forum.
Who else will be able to access my contact details?
The Forum is managed by a committee formed from our patients. They will hold contact details, address, phone and email address, for the members but no clinical or other personal information will ever be shared with the Forum committee.
How often will you contact me?
You will be sent the agenda for the next meeting and minutes of the past meeting. You may also be sent additional information as appropriate
Do I have to take part in the group?
No, but if you change your mind, please let us know.
What if I no longer wish to be on the contact list or I leave the surgery?
We will ask you to let us know if you do not wish to receive further messages. If you leave the practice, you will no longer be eligible to remain on the group.
